IMPORTANT UPDATE:
Use the Vital Statistics Unit's new birth and death certificate online ordering system.
The City Port Arthur Health Department now offers a faster way of obtaining vital records such as birth and death certificates.
The office recently partnered with Permitium, an online portal that provides a more convenient service option for residents and non-residents by allowing them to apply and pay for vital records online. The process for receiving vital records will involve:
- Applying through the portal
- Providing required documents and identification
- Finalizing with payment
- Receiving the certificate through mail
Those who use the online ordering system will also have the option to pick up their records at the City of Port Arthur Health Department.
The Vital Statistics Unit maintains vital records for the state of Texas, including:
- Birth and death certificates
- Fetal death records
Forms:
- Birth and Death Application
- Affidavit of Personal Knowledge Form (Out of Town Applicants)
- Application for Certified Copy of Birth or Death Certificate.
You must include Affidavit of Personal Knowledge with an application.
