Vital Records

The mission of the Vital Records office is to collect, protect and provide access to vital records and vital records data to improve the health and well-being in Texas.

PLEASE NOTE: 

Due to Covid-19, the Health Department has taken safety  precautions to protect the staff and clients.  At this time the Vital Statistics Department has implemented an appointment-only schedule.  No walk-ins will be accepted at this time. Contact the Registrar’s Office at (409) 983-8850 or (409) 983-8853 to schedule an appointment. Please be sure to arrive on time when you are scheduled to enter the building. We apologize for any inconvenience.

The City of Port Arthur Vital Statistics Unit maintains vital records for the state of Texas, including:
  • Birth and death certificates
  • Fetal death records

Updated - Revised Polices in Response to Supreme Court Ruling

The Department of State Health Services (DSHS) is implementing revised policies and will also amend certain vital events forms and records in response to the June 26, 2015 Supreme Court ruling in Obergefell and the July 7, 2015, U.S. District Court ruling in DeLeon. The revised policies have been updated (as of August 24, 2015). For additional information visit the DSHS website.
