    1. Please enter Contact person's name.
    2. Please enter the Contact person's phone number.
    3. Please enter the number of banner(s) to be installed.
    4. Please enter the event/project description.
    5. Rules/Requirements
      1 - Banners are to be dropped off/picked up from City Hall, 444 4th St., Suite 321, one week prior to installation and within seven working days after removal.
    6. 2 - Banner installation will be limited to a two week period starting on Monday and ending on Sunday.
    7. 3 - Entire area of banner must be made of 1/4" or larger mesh material, excluding letters and dates, so as to allow for wind to pass through banner. Tight mesh banners or those with wind flaps cut into them will NOT be hung.
    8. 4 - All banners must be a maximum of 36' wide and 3' tall.
    9. 5 - All banners must be equipped with flag-style clips attached at both the bottom and top.
    10. 6 - All banners must be equipped with four 6' nylon ropes attached to each corner.
